CC Kid's Volunteer Application

This form is to be completed by all adults involved in the supervision or custody of minors. To facilitate a safe and secure environment for all children, students, volunteers; it is necessary to gather pertinent information from those who work within our ministries. This information will be used for the sole purpose of helping Christ Chapel Church NextGen team to provide a safe and secure environment for all children, students, volunteers, staff and families of Christ Chapel Church.
